Closes #60

The env var token now reads and creates issues (#57-#60 were created with it), so CLAUDE.md now names it as the default. The dotfile is kept only as a fallback for older containers.

The env var was re-provisioned with the issue-scoped key, so it is now
the default for Issues/PR/release calls. The dotfile stays documented
only as a fallback for older containers.
